We request that you provide our office with a minimum of 24 hours notice for appointment cancellations. If you do not provide 24 hours notice for your appointment, you may be charged. As a patient if you miss three or more consecutive appointments without 24 hours of notice, you will only be accepted for same day treatment, schedule willing.
We accept any insurance that will allow you to choose a provider of your choice and are a networked provider for most insurance companies including but not limited to: Anthem BlueCross & BlueShield, Cigna, Delta, Humana, Indiana
HIP, Medicaid, Metlife, United Concordia, and United Healthcare.
